Under the direction of the Senior Director, the Multidisciplinary Team (MDT) Coordinator provides enhanced coordination of cases with the Child Advocacy Center Multidisciplinary Team to ensure timely resolutions, efficient communications, and information sharing. The MDT Coordinator also provides support in direct service, consultation, and case management to the Senior Director and the clinical forensic team.

 

Responsibilities: 

  • Collaborates with the multidisciplinary team (MDT) to ensure that all investigations and services are coordinated efficiently and expeditiously;
  • Ensures that the MDT is aware of incoming cases referred to the Center for forensic services, that the MDT team collaborates on the cases and that the family is kept informed;
  • Collaborates with the Senior Director in preparation of case files and agendas for MDT weekly Case Planning or Expedited Case Review meetings, Case Review meetings, and facilitates these meetings as needed;
  • Collaborates with the Senior Director in collecting and reviewing data, and ensuring accuracy of statistical management reports and accurate completion of funder reports;
  • Assumes a primary role in responding to NCATrak data requests in close collaboration with Senior Director & Safe Horizon’s Research & Evaluation Department;
  • Assists the Clinical Team’s Supervisors in ensuring the coordinated provision of on-site mental health services and referrals for off-site services by the clinical team;
  • In collaboration with the clinical team, advocates on behalf of the child victim and non-offending family members concerning case management needs, domestic violence assessments, investigation coordination, and  medical and mental health services;
  • Liaises with the CAC medical team to ensure the provision and coordination of medical services to child victims;
  • Liaises with the CAC prosecutorial teams to coordinate information sharing regarding case status, court dates and appointments with family members;
  • Serves as the point person for the New York State Office of Victim Services (OVS) by overseeing the proper completion and submission of crime victims benefits applications to OVS and following up with OVS to ensure submission of missing data, and to confirm benefit denials and awards;
  • Works collaboratively with the clinical team and Safe Horizon staff to research, complete and secure emergency client assistance for clients through Safe Horizon’s Emergency Client Assistance Program (ECAP) and/or Project Debby;
  • Works collaboratively with Safe Horizon and other programs to facilitate linkages for clients;
  • Represents Safe Horizon at client conferences facilitated by the New York City Administration for Children’s Services (ACS) and/or Foster Care Agencies to ensure the coordination of mental health, victim advocacy, medical and forensic services through the CAC;
  • May supervise undergraduate interns as assigned;
  • Various other duties and special projects to be assigned by the Senior Director.

 

Qualifications:

Required: 

  • Bachelor’s degree in social work, counseling, human services or related field or equivalent training or experience; Master’s degree is preferred;
  • 2-3 years of professional experience working with children and families who have experienced abuse and trauma, and/or general mental health practice;
  • Ability to work effectively as a member of a multidisciplinary team with a willingness to appreciate and value different points of view;
  • Excellent organizational and time management skills required
  • Strong interpersonal relationship and crisis management skills;
  • A sound knowledge of child trauma and victimization issues;
  • Constructive problem solving and conflict resolution skills;
  • Excellent interpersonal, verbal, and written communications skills;
  • Proficiency in excel, and ideally NCATrak

 

Work schedule:  

Tuesday to Friday 1pm to 9pm

Saturday 10am to 6pm
